We’re continually working to strengthen and support collaborative family practice teams across the province. You become a patient of a collaborative family practice team when you get accepted as a new patient by a family doctor or nurse practitioner who works as part of a team, or if your family practice transitions to become a collaborative family practice team.
If you’re unsure if your primary care provider is part of a team, ask your family doctor or nurse practitioner.
Need a Family Practice registry
Having access to a primary care provider is important to all of us for our health and wellness. If you don’t have a family doctor or nurse practitioner, please register online with Need a Family Practice. You can also register by phone by calling 811, Monday through Friday, between 10 a.m. and 6 p.m.
Many collaborative family practice teams accept new patients from the registry. We encourage all family doctors and nurse practitioners in Nova Scotia to contact the health authority for a list of names of people who are looking for a family practice. People are connected with a family practice based on when they have added their name to the registry. Primary care providers who are accepting new patients may work as part of a collaborative family practice team, or in a group or solo practice setting.
